POCA Nomination Criteria and Form
The deadline for 2012 nominations is now closed

Nominations are sought for youth in Grades 6 to 11 who are making an extraordinary difference in the lives of others.
The nominator and nominee must collaborate in preparing the application to assure completeness and accuracy.
Forms may be submitted separately as long as both are received by the deadline.
A complete entry form must include the following:
- Nomination Form completed by nominator that separately addresses each of the four categories of award criteria.
- Nominee Statement completed by nominee that separately addresses each of the four categories of award criteria, including a Nominee Agreement completed by nominee and parent or guardian.
NOTE: A nominator is someone who can attest to the extraordinary project the nominee has completed. A nominee is someone who has made a difference in the lives of others through extraordinary efforts in creating his or her project.
